The primary difference between the 148 and 148 EXTENDED vans is the 148 EXTENDED is about 28-inches longer behind the rear wheels. The EXTENDED body is only offered in the high roof configuration, so if you have a mid- or low-roof, you do not have the EXTENDED body.
